Dr. Cusack received his baccalaureate
degree from the University of Notre Dame (1964), his M.D.
degree from the University of Illinois at Chicago (1967),
completed his residency in Diagnostic Radiology at Kansas
University Medical Center (1973) and is Board Certified in
Diagnostic Radiology by the American Board of Radiology
(1974). He is also board certified in Medical Management by
the American Board of Medical Management. Dr. Cusack is a
Diplomat of the National Board of Medical Examiners, an
Associate of the American College of Healthcare Executives,
and a Certified Professional of Healthcare Quality by the
National Association for Healthcare Quality, and a
Board-Certified Physician Executive by the American Board of
Medical Management. Dr. Cusack is a
member of numerous professional societies: American Roentgen
Ray Society, Association of University Radiologists,
International Society for Diagnostic Ultrasound in
Ophthalmology, Society of Chairman of Academic Radiology
Departments, American College of Radiology, American
Institute of Ultrasound in Medicine, Radiological Society of
North America, American Medical Association and Illinois
Medical Society. In 2002, Dr. Cusack was instrumental in
gaining the approval for the radiology department to double
the size of their residency program.
Dr. Cusack is Professor of Clinical Radiology
and has served as Chair of the Department of Radiology at
the University of Illinois College of Medicine at Peoria
since 1983. Dr. Cusack's solid background and expertise in
the areas of clinical radiology and medical education were
instrumental in the development and establishment of the
Diagnostic Radiology Residency program in 1992, the Breast
Imaging Fellowship in 1993, a Neuroradiology Fellowship in
1997, and a Vascular/interventional Radiology Fellowship
Program launched on July, 1998. In recent past, Dr. Cusack
has been honored to be chosen to provide the Convocational
address for the University of Illinois College of Medicine
at Peoria graduating Classes of 1990, 1998, 1999 and 2003,
provided the invocation at numerous convocation ceremonies,
has been a six-time recipient of the Raymond B. Allen Golden
Apple Teaching Award (chosen by vote of the senior medical
students), a recipient of the Faculty of the Year Award
which is awarded annually by the faculty of the University
of Illinois College of Medicine at Peoria and a 1997
recipient of the Faculty of the Year Award for all four
University of Illinois College of Medicine sites. In May,
1997, Dr. Cusack was appointed Regional Vice - Dean of the
University of Illinois College of Medicine at Peoria and
served in that position from 1997 to 2002. |
